North Shore-LIJ Partners with Two Suffolk Skilled Nursing Care and Rehab Facilities
New Affiliation Agreements Improve Continuum of Care for Seniors
To strengthen healthcare services for seniors in Suffolk County, the North Shore-LIJ Health System today announced the signing of clinical affiliation agreements with the Carillon Nursing and Rehabilitation Center in Huntington and the Huntington Hills Center for Health and Rehabilitation Center in Melville.
“These new affiliations increase our ability to collaborate with healthcare professionals at two outstanding nursing care and rehabilitation facilities to enhance clinical care for seniors and develop geriatric medical programs,” said Michael Dowling, president and chief executive officer of the North Shore-LIJ Health System. “As Long Island’s largest healthcare provider, North Shore-LIJ looks forward to sharing resources and expertise with these two new facilities to provide a continuum of care for seniors in Suffolk County and help address the diverse needs of the growing population of aging baby-boomers in our region.”
Depending on the needs of each facility, services North Shore-LIJ will provide include: rapid intervention and referrals to specialists, consulting services for quality management/quality improvement, home care services, hospice care services, palliative care/end-of-life care and geriatric medical education programs.

“For several years, Huntington Hills Center has worked with many of the providers of the North Shore-LIJ Health System. Our long-term, rehab and sub-acute programs complement their network continuum of services,” said Kenneth Knutsen, administrator of the 320-bed Huntington Hills Center for Health and Rehabilitation Center. “This affiliation will improve coordination, communication and education between our organizations, which will promote optimal care for all Long Islanders we will collaboratively serve.”
About Carillon Nursing and Rehabilitation Center Located in Huntington, NY, Carillon Nursing and Rehabilitation Center is a 315-bed skilled nursing facility. The facility also has an independent short-term unit that offers a variety of sub-acute services including physical and occupational therapy in a newly renovated state-of-the-art rehabilitation room offering out-patient rehabilitation services as well. Carillon has a 60 registrant medical model adult day care program, including door through door transportation, and recently opened an onsite renal dialysis center. Carillon Nursing and Rehabilitation Center is a one floor facility with spacious grounds, outdoor courtyards, and many amenities to make our residents feel at home.
About Huntington Hills Center for Health and Rehabilitation Center Huntington Hills is a 320-bed skilled nursing facility in Melville, NY that provides long term skilled nursing care as well as short term sub-acute rehabilitation and a medical model adult day care program. The center’s staff includes licensed nursing professionals, social workers, recreation therapists and dieticians. The medical staff is comprised of board-certified physicians from area hospitals. Huntington Hills was designed to reflect a hotel-like atmosphere and includes high-end furnishings and decorations, providing high-quality residential healthcare living. Amenities include wireless internet access, a beauty shop and multi-faith chapel.
