Becoming a Rehabilitation Patient
Patients usually come to Southside for rehabilitation from another hospital. When patients anticipate rehabilitation after elective surgery, they should discuss those needs before surgery and get a referral from their doctor at that time. Following an unexpected injury that requires rehabilitation, patients can be referred to Southside by their doctors. After patients have been referred to Southside, social workers or discharge planners at the referring hospital will work with Southside to make the transfer arrangements. After admission, patients will be examined by one of the physiatrists, and a rehabilitation plan will be developed.
To arrange a transfer to Southside Hospital for rehabilitation, the referring hospital can call the Department of Physical Medicine and Rehabilitation at (631) 968-3330.
Families and potential patients anticipating rehabilitation are encouraged to tour Southside's rehabilitation facilities. Please call (631) 968-3446 for an appointment.
If your doctor has prescribed physical therapy, the Institute for Physical Therapy can be reached at (631) 968-3400.
