Overview:
Current trends in the economy, health care industry, medical and services delivery and most importantly, patient preferences have contributed to the estimated thirty-four million family caregivers in the United States today. Rarely are they prepared, trained, supported or even acknowledged. Designed to target the many faces of family caregivers - those caring for a spouse, parent, sibling, or an adult child - this conference will focus on the common issues among caregiver and offer education, support and practical assistance. Whether being close at hand, an on-site or a long-distance caregiver, this FREE conference will provide practical strategies and solutions for caregivers as well as providing attendees with the opportunity to meet and network with other caregivers in similar situations. Participants will have ample time to obtain resources and referral materials at exhibit area.

Audience:
Family caregivers, patient representatives, representatives of patient/family advocacy and faith-based organizations, care managers, social workers, nurses, hospice, home care and community-based providers, human resource professionals.

Directions:

LONG ISLAND EXPRESSWAY:
Eastbound: Exit at Easthampton Boulevard/Douglaston Pkwy. (exit 30). After the traffic light, DO NOT enter Cross Island Parkway, but take the next right southbound, which is Douglaston Parkway. Continue a half-mile to the Center, which will be on the left side of the road.

Westbound: Exit Little Neck Parkway (exit 32). Take service road to Douglaston Parkway. At the traffic light, make a left turn and continue on Douglaston Parkway south bearing right, passing the shopping center and at the bottom of the hill DO NOT go uphill toward the parkway, but turn left, continuing on Douglaston Parkway, past the housing development on the left. Continue a half mile on Douglaston Parkway to the Center on the left side of the road.
