The Health Information Management Department supports Forest Hills Hospital's goal of being a prominent provider of quality healthcare services, a sponsor of medical educational excellence and a leader in clinical research via the quality of clinical data that it maintains.
Requesting Records
Copies of records are available for patients and physicians with the proper authorization. The hospital has the right to charge 75 cents per page for copies of medical records. Proper release of information forms may be retrieved from the Health Information Management Department. Please contact the correspondence secretary at (718) 830-4197. Physicians may obtain records for patient care Monday through Friday from 7 a.m. to midnight and Saturday, Sunday and holidays from 9 a.m. to 5 p.m.by calling (718) 830-4190. Please contact the nursing supervisor on shift at (718) 830-4000.
