Franklin Hospital's Adult Day Health Care Program Expands to Seven Days
Franklin Hospital is proud to announce that its Orzac Center for Extended Care and Rehabilitation is expanding the Adult Day Health Care Program (ADHC) to seven days a week. The program had previously been a five-day program. ADHC is a community based long-term program in which the participants spend 9 a.m. to 3 p.m. at Orzac and then return home. It is designed to keep the frail and elderly or the chronically ill/disabled adults who live within a 15 mile radius of the ADHC program in their own homes with dignity and respect.
The program is offered in a friendly and warm group setting at Orzac, 900 Franklin Avenue in Valley Stream. The ADHC services include: door through door transportation, nursing assessments and treatments, personal care/ADL, physical and occupational therapy, dental, social work case management, therapeutic recreation activities, nutritional counseling, health education, continental breakfast and nutritious lunch and snacks.
The goal of the program is to help the participants maintain wellness and to work with each participant's personal physician to prevent untimely long term placement and utilization of acute settings such as emergency rooms, etc. The ADHC program is New York State Medicaid funded. Those who are not Medicaid eligible may pay directly. The number of days a participant takes advantage of the program is arranged on an individual basis.
