You can download a Health Care Proxy in English, Spanish, Chinese or Russian to fill out. The Health Care Proxy will be honored by hospitals, doctors and othe health care professionals in New York State. Give a copy to your health care agent, your physician and other health care providers, your attorney and any other family members or close friends.
