Transitions of Long Island is committed to providing the highest quality care for individuals with acquired brain injuries, and supporting our clients throughout their rehabilitation and beyond, promoting their independence and productivity within their communities.Our team of compassionate, highly-trained staff are dedicated to providing specialized, state-of-the-art, holistically-oriented treatment, in order to respond effectively to the complex needs of the individuals whom we serve.

Consumer-Driven
We work as a team, and you and your family are an important part of our team! Your input is vital to the rehabilitation process. We use it to customize your program so that it best suits your individual needs, to keep improving our quality of care, and to create the most stimulating, challenging and supportive environment in which to recover.You are always encouraged to bring comments, concerns and suggestions to program staff.We will formally ask for your input on surveys distributed during your stay and after your discharge.Our Consumer Satisfaction Committee is dedicated to reviewing your input and making positive changes, whenever feasible, in accordance with your feedback.

Outcome-Oriented
The primary focus of rehabilitation efforts at Transitions is to create a program to maximize your recovery, and help you to achieve your desired goals. Unfortunately, this typically doesn't happen overnight, or in a week, or a month.It takes time to recover following a brain injury, and Transitions emphasis is on guiding clients to return to home, work and/or school activities gradually at the right time and pace. By the time of discharge from outpatient program, it is anticipated that the client will be successfully re-integrated into productive, community-based activities (e.g., homemaking, work, school, volunteering, etc.).

Adaptable
Transitions is committed to providing services which will meet your own individual needs. In order to do so, we strive to be flexible, and adapt to clients with different types of brain injuries, problems and circumstances. Your therapy schedule, designed by you and your therapists to target your specific needs, will be comprised of a combination of individual and specialized group sessions.As you progress, your schedule will change, to meet you at the level of challenge you're ready to take on during each phase of your recovery.

Comprehensive
Transitions strives to be your central guide, or resource, on your path to wellness. We hope to provide most of the services which you might need during your recovery. These might include physical, occupational,speech-language therapies, individual, group or family counseling, and cognitive rehabilitation. When you need a service which Transitions does not directly provide, we will refer you to an appropriate resource, and make sure this service is integrated within your overall treatment plan. For example, we work with schools/tutors, employers, vocational rehabilitation agencies and specialist physicians (e.g., neuro-optometry, neuro-psychiatry, neuro-urology), and consider these providers as integral members of you 'team'.

Holistic
At Transitions, clients are viewed as fellow persons, not just as clients. We are committed to focusing and capitalizing on strengths, rather than weaknesses; on what can be achieved versus what can't. Our philosophy toward rehabilitation is that the person served needs to be treated holistically- which means not only addressing a person's physical, cognitive and/or emotional problems resulting from their injury, but developing an understanding of the person as a social and spiritual being; considering the person's role in the context of their family and community; and considering the interactions between each of these aspects in finding the best path toward recovery and achievement of one's goals.
