Case Management
Case management promotes rehabilitation through the provision of care and services to an identified psychiatrically disabled population of children, adolescents and adults. Case management services are intended to assist consumers in gaining access to needed medical, psychiatric, social, vocational and other services which may be essential in meeting basic human needs.
Intensive case management and supportive case management are provided under a centralized model through the Nassau County Dept. of Mental Health, Mental Retardation and Developmental Disabilities in cooperation with the L.I. Regional Office of the NYS Office of Mental Health. In addition to this type of case management service, outpatient clinic programs in Nassau County provide case management services to consumers actively receiving aftercare through the specific clinic.
