Comprehensive Ophthalmology

The Comprehensive Ophthalmology Service performs routine ophthalmic evaluations that screen and treat a wide range of ophthalmic conditions including cataracts, dry eye syndrome, glaucoma, diabetic retinopathy, and macular degeneration.

Cornea and External Disease

Dr. Ira Udell is Chief of the Cornea and Anterior Segment Division and Chairman of the department of Ophthalmology. The Cornea & External Diseases service provides comprehensive consultation and treatment of corneal disorders including corneal infections, keratoconus,dystrophies, scarring disorders, dry eye, cornea irritation, allergic conjunctivitis, Blepharitis, Episcleritis, Scleritis, trauma, post cataract complications, external diseases, conjunctival tumors and inflammation. Specialized surgical procedures include corneal transplants, DSAEK (Descemet's Stripping Automated Endothelial Keratoplasty) complicated pterygium excisions and management of ocular surface disorders (i.e. ocular cicatricial Pemphigoid) and tumors. Outpatient evaluations are available at the Department of Ophthalmology in Great Neck.

Cornea, Cataract, and Refractive Surgery

Carolyn Y. Shih, MD, MBA, MPH, is the Co-Director of the North Shore-LIJ LASIK Center. As a specialist in corneal and external diseases, cataract surgery, and laser vision correction, Dr. Shih performs corneal transplants, Descemet’s Stripping Automated Endothelial Keratoplasty (DSAEK), Phacoemulsification (sutureless cataract surgery), Anterior Segment Reconstruction, (including sutured intraocular lenses), and laser vision correction (LASIK, PRK).

Adult Strabismus

Adult patients with strabismus (misalignment of the eyes: esotropia, exotropia, hypertropia, fourth nerve palsy, sixth nerve palsy, third nerve palsy) are treated by our Pediatric Ophthalmology Service by referral only, usually after they are fully evaluated by their Comprehensive/General Ophthalmologist or Neuro-Ophthalmologist.

Glaucoma

Glaucoma is a group of eye diseases affecting the optic nerve, the transmission cable which sends images to the brain. It can affect patients of all ages, although its occurrence in children in unusual. With over a million nerves, damage usually occurs gradually and steals sight without warning, often without symptoms. High intraocular pressure (IOP) is one of many risk factors which influence the loss of optic nerve tissue. The glaucoma service evaluates patients with glaucoma, and those in whom glaucoma is suspected.

Using state of the art diagnostic instrumentation including computerized visual field testing, optic nerve tomographic scanning (O.C.T.), corneal thickness measurement and biomicroscopic evaluation of optic nerve and outflow channels, patients are individualized for appropriate treatment. Treatment modalities for glaucoma include topical (eye drops) laser, microsurgical (glaucoma filtering and glaucoma drainage device) procedures. Members of the glaucoma service are actively involved in the training of residents in the department's program. The members of the service are actively involved in clinical research in the treatment of glaucoma and participate in national and international collaborative studies to search for new treatments for this significant cause of visual loss.

Oculoplastics and Orbital Disease

Disorders of the eyelids, the lacrimal (tear duct) system, and orbit (the eye socket) are evaluated and treated by our closely-allied Oculoplastic surgeons. Typical problems they treat are excessive tearing, eyelid abnormalities, (ptosis or drooping), and severe cases of thyroid eye disease (Graves' disease). Vitreo-Retinal Disease

Dr. David Fastenberg is Chief of the Vitreo-Retinal service. Patients in this subspecialty include those with age related macular degeneration and diabetes mellitus, among others. Retinal examination will often include the performance of digital fluorescein angiography in order to fully assess the retinal and choroidal circulations. Some patients with diabetic maculopathy and age related macular degeneration may benefit from laser photocoagulation. Both green and red diode lasers are available on site. Other inflammatory and infectious diseases of the retina are treated, including opportunistic infections in patients with AIDS. Patients with Retinitis Pigmentosa are evaluated and counseled.

Uveitis and Ocular Disease

The service provides comprehensive consultation and treatment for all forms of ocular inflammation. Disorders treated include both infectious and autoimmune diseases. They include systemic diseases such as sarcoidosis, juvenile rheumatoid arthritis, Lyme disease, TB, syphilis; as well as diseases limited to the eye such as toxoplasmosis, acute retinal necrosis, and idiopathic etiologies.

Low Vision

What is Low Vision? An individual is said to have low vision when eye diseases such as macular degeneration, cataracts or glaucoma reduce the clarity and quality of their vision, despite the fact that they may be wearing the best possible spectacle correction. Many people with low vision can often be helped with the use of special glasses or magnification systems, both for distance and near visual for tasks such as watching television or reading the newspaper.

The NS-LIJ Department of Ophthalmology‘s Photography Division features state of the art ophthalmic imaging that provides photographic and digital documentation for patient care, clinical research, and vision research activities. Our ophthalmic photography service performs digital photography for both clinical and research purposes and furnishes the faculty, residents, and fellows with prints and PowerPoint presentations for publication and teaching purposes respectively.
