Social Work at Syosset

Using their knowledge of: human behavior, family structure, the impact of illness and disability on people, available community resources and hospital services, social workers participate in activities designed to help patients, their families, the staff and the community solve problems related to illness, disability or hospitalization.

Services Provided

Psychosocial assessments are performed on all patients that meet the high-risk criteria, as well as those that are referred during the hospital course. The Social Work Department is an integral part of the interdisciplinary team, and participates in multidisciplinary team rounds. It is responsible for counseling services, crisis intervention, discharge planning and linkage to community resources including adult home and assisted living placement, skilled nursing facility placement, rehabilitation placement (physical, psychiatric, substance abuse), and child/adult protective services referrals. Social Workers also assist patients and families with end of life issues and advance directives.
