Case Management
Case management promotes rehabilitation through the provision of care and services to an identified psychiatrically disabled population of children, adolescents and adults. Case management services are intended to assist consumers in gaining access to needed medical, psychiatric, social, vocational and other services which may be essential to meeting basic human needs.
