System Rehabilitation Services
In order to achieve the highest level of function in the timeliest manner possible, you may need additional care after you leave the hospital to safely function at home. Understanding and planning for this care now can help reduce the length of time you have to spend in the hospital and make sure you gain access to the highest quality, most comprehensive after-care possible.
Rehabilitation refers to the restoration of skills and abilities needed for you to function as independently as possible. The North Shore-LIJ Health System is committed to providing the highest quality of care and patient outcomes for those with musculoskeletal and neurological disabilities. Depending on your specific symptoms and needs, you may benefit from care provided at different levels or in different settings. For example, a brain injury or stroke can limit your ability to walk, talk, eat, bathe yourself or affect your memory and mood. In order to maximize your recovery these impairments require the care of a specialized comprehensive rehabilitation team.
A multidisciplinary team of rehabilitation specialists will develop an individualized plan of care for patients who need rehabilitation. This may be in a specialized inpatient rehabilitation unit, an outpatient center, or in your home. Your doctor and therapists will recommend the level of rehabilitative care that will best suit your needs. Your discharge planner/care coordinator will review these options with you and your insurance company so that you can make an informed decision about your continued care.

The North Shore-Long Island Jewish Health System believes that the community should have information as the outcomes of each program within the system. Each item is important in the decision of each family and individual seeking services.
Percent Discharge to Home:
This measurement shows how successful a facility is in getting people home after their stay in an inpatient facility.
This is influenced by a number of factors including:
- the resources available to the families in the communities in which they live which can support the person at home etc.
- the severity of cases that each facility takes i.e. the more involved a person is neurologically the more they may need care after their inpatient rehabilitation stay. In general the greater the percentage of orthopedic patients in each facility the greater the percentage of patients that are discharged home. The percentage of patients discharged home from January-March 2008 is as follows:

The volume of patients that come through a program gives a sense of how active the program is and is based on the number of available
beds for patients. The more beds a facility has the larger the number of overall yearly discharges will be.

Likelihood to recommend the program:
