The Zucker Hillside Hospital Ambulatory Care Pavilion Geriatric Program – 2nd floor Glen Oaks, NY 11004 718-470-8140 The Hillside Geriatric Center provides comprehensive psychiatric evaluation and treatment to older adults. Typically patients seen at the center include those with psychiatric disorders and memory disorders and dementing conditions with associated behavioral disturbances. Many patients have psychiatric/neuropsychiatric symptoms related to underlying medical conditions. The Center primarily serves patients 65 years of age and over, but also accommodate younger patients with diagnoses/problems that are consonant with older adulthood, e.g., presenile dementia.

       8:30 am to 5:00 pm – Monday through Friday

Insurance Accepted:
       Medicaid, Medicare and most private insurances are accepted. In addition, a sliding scale fee may be arranged.

Directions:
       Car: (From Manhattan, Westchester and New Jersey) Long Island Expressway or Grand Central Parkway (East) to Little Neck Parkway Exit, Turn right (south) on Little Neck Parkway and Proceed to Union Turnpike to 263rd Street; left two blocks to 74th Ave, turn right and proceed straight ahead to the entry to the Ambulatory Care Pavilion grounds. (From Long Island) Long Island Expressway or Northern State Parkway (West) to Lakeville Road South Exit, Turn left (south) on Lakeville Road and go to Union Turnpike. Make right at 263rd St; proceed two blocks to 74th Ave, turn right and proceed straight ahead to the entry to the Ambulatory Care Pavilion grounds.

      Bus: The Q46 rides along Union Turnpike. Get off at 263rd Street and walk up north to 74th Ave., Make right and proceed straight ahead to the entry to the Ambulatory Care Pavilion grounds.
