What is an Advance Directive?
An advance directive (i.e., health care proxy) is a legal document in which you specify what type of medical care you want in the future or who you want to make healthcare decisions for you if you become medically unable to make decisions. An advance directive allows you to maintain control over your own medical care, even at a time when you cannot communicate for yourself.
