Transitions of Long Island® is a comprehensive neuro-rehabilitation program for individuals with acquired neurologic injuries, including traumatic brain injury, stroke, brain tumor, anoxia, encephalitis, mild head injury and other brain diseases. A program of the North Shore-Long Island Jewish Health System, Transitions® has experience in providing specialized services to individuals with ABI since 1988.
Services offered:
- Neuropsychological evaluations
- Individual and group counseling
- Family education and support
- Physical therapy evaluations and treatment
- Occupational therapy evaluations and treatment
- Speech/language therapy evaluations and treatment
- Prevocational training
Specialized services include:
- Saeboflex upper extremity orthotic system
- Bioness upper and lower extremity neuro prosthetic devices
- Electrical Stimulation, VitalStim therapy, fordysphagia
- Lee Silverman Voice Treatment (LSVT) for Vocal Disorders
- Vestibular Rehabilitation
- Adolescent Community and School Re-Entry Programs
- Structured Day Program through MedicaidWaiver to develop prevocational, socialization and cognitive skills.
Post Discharge Services:
- Post-Rehabilitation Extension Program (PREP) - to allow clients to reinforce skills learned in therapy and provide continuedstructure and stimulation
- Alumni group - monthly wellness series
- Referral to various volunteer opportunities
- Referral to appropriate vocational programs
