Adult Day Health Care
The Adult Day Health Care program's mission is to establish, maintain and provide outpatient services in the continuum of care for the frail and elderly, chronically ill adults, and the developmentally disabled. Program Registrants continue to live in their homes, in their neighborhoods.
We strive for excellence in every aspect in the delivery of that care. This is possible through the dedication and ongoing performance improvement of the program's staff.
Orzac CECR's Adult Day Health Care Program, a medical model adult health care offering, is uniquely situated in a medical center setting. The program provides and arranges for comprehensive health care service and socialization in a congregate day setting located in the Orzac Center for Rehabilitation and Extended Care.
We service registrants from Nassau and Queens counties. Attendance may vary from one to six days a week, based on assessed need.
Our program is staffed with medical professionals, including licensed registered nurses, NYS certified nurse aides, professional physical and occupational therapists, social worker, therapeutic recreation therapist, and a registered dietician. The Adult Day Health Care program is sponsored by Medicaid or Private Pay.
The services available are:
- Comprehensive health services (nursing, dental, podiatry, nutritional counseling, ophthalmology, lab, x-ray, EKG)
- Rehabilitation Services
- Social Work Case Management
- Therapeutic Recreational activities throughout the day
- Continental breakfast, full lunch, snacks as needed
- Door-to-door transportation provided by ambulette
- Unisex hair services are also available
- Health Education
- Personal Care
