The North Shore-LIJ Home Care Network's Intensive Oncology Home Care Program focuses on the cancer patient's need for Home Care for periods of time throughout the course of their disease.This need could be directly related to the cancer, the treatment of the cancer or a pre-existing condition that has become a problem because of the cancer.The Intensive Home Care program can, when needed, help the patient in the transition to a certified hospice program.
Intensive Home Care Team
- Managed by an Oncology Certified Nurse, under the direction of the patient's physician
- Medical Director is a board certified Hematologist/Oncologist available for consultation and staff development
- Social Worker specializing in Oncology available for consultation on concrete services and emotional support
- Services of an experienced team of Visiting Nurse, Physical Therapy, Occupational Therapy, Speech Therapy, Wound Ostomy nurse specialist, Infusion Services and Home Health Aide are provided as needed
