Long-Term Home Care
The program is often described as a "Nursing Home Without Walls" or the "Lombardi Program."
The program offers a wide range of individualized services to disabled or chronically ill home bound patients, assisting these patients to maintain their place in the community. Professional staff visit regularly and provide coordinated, ongoing care and assessment while maintaining communication with the patient's physician.
Services are reimbursable under Medicaid with the approval of the local Department of Social Services.
