SHORT TERM (SUB-ACUTE) REHABILITATION SERVICES

Healing your body, reclaiming your life
Our center is dedicated to providing result-oriented, comprehensive inpatient rehabilitation services. Our goal is to maximize your independence, increase your function and improve your quality of life.

We provide complete rehabilitative care in a short-term setting for individuals recovering from surgical procedures, amputation, stroke, trauma, spinal cord and brain injuries and for conditions such as arthritis, cancer, stroke and Parkinson’s disease.

We know that each of our patients has a unique set of personal circumstances. Our team of nursing and rehabilitation professionals work together to set goals and design a treatment program that fits your special needs. Depending on your condition and goals, your personalized team may include:

  • Physiatrists (doctors who specialize in physical medicine and rehabilitation)
  • Other Specialists
  • Nurses
  • Therapists (occupational, physical, speech, recreational)
  • Psychologists
  • Geriatricians
  • Primary Care Physicians
  • Care Managers

Your rehab team provides care under the direction of your primary physician, and your primary physician will receive regular updates about your progress. Your family and friends are encouraged to visit and participate in your rehabilitation, as well as attend patient/family conferences about your progress and your at-home care after discharge.

ADULT DAY HEALTH CARE PROGRAM

A daytime home away from home
The Adult Day Health Care program offers a safe and caring home away from home for your loved one during the day. The program offers frail, elderly, chronically ill and disabled adults a supportive environment where they will have the opportunity to make friends and explore hobbies seven days a week. It also gives you the time you need to work, run errands or enjoy some important time for yourself. Because of its unique and personalized services, our program can help your loved one live at home longer and serves as a bridge to all the services he or she needs, including:

SKILLED NURSING (NURSING HOME) SERVICES

Comprehensive care for the highest quality of life
When nursing, medical care and supervision are needed consistently for your aging loved one, we will provide a personalized care plan designed by a team of skilled professionals. Together, you and the skilled nursing team will make decisions on treatments that maximize function and quality of life and respect your loved one’s personal preferences. The team welcomes questions and helps you and your loved one understand all the issues related to care and treatment options.

The team, guided by a staff physician, provides updates to your loved one’s personal physician. Depending on individual needs, consultations can be arranged with specialists such as orthopedic surgeons, neurologists, psychiatrists, physiatrists, podiatrists, ophthalmologists and dentists. The care team will also consist of skilled and compassionate nurses, therapists and other healthcare professionals offering a range of specialized services including:

  • Physical therapy
  • Occupational therapy
  • Wound care
  • Wound VAC
  • Intravenous therapy
  • Transfusions
  • Tracheostomy care
  • Oxygen therapy

We know that delicious food, good service and menu choice are important to your experience. A registered dietician will assess your individual nutrition needs and preferences to ensure healthy, tasty meals that fit the needs of your diet – and your taste buds. Over 30 menu choices are offered each day. Holidays and special events are celebrated with foods appropriate to the occasion.

Social Work

Your planning and preparation
Social work and discharge planning services are available to help in a number of ways, including:

  • Supportive counseling
  • Discharge planning (transition to community or alternative living arrangement)
  • Guidance on treatment decision-making and preparing advance directives
  • Education regarding community and government resources

Activities

Music, games, crafts & more
Each day, organized activities and social opportunities are provided to promote creative expression, diversion and relaxation. Depending on your unique needs, you or your loved one can participate in music, games, crafts, socials, trips and exercise programs. Family members are encouraged to join in activities and pets are frequent visitors.
