North Shore-LIJ is pleased to offer FollowMyHealthTM, a patient portal that allows you to conveniently manage your personal health information – anywhere, anytime. This free, self-service health management tool can improve your interactions with our health system by giving you access to important information about your doctor and hospital visit. By creating an account, you will be able to take a more active role in managing your health and wellness, giving you the ability to make more informed decisions about your health.
Who can participate?
At this time, FollowMyHealthTM is available to Medical Group patients at select practices and all North Shore-LIJ Health System hospital patients 18 years and older.
Proxy access is also available to parents and guardians of children under the age of 13.
Currently, health information is available through FollowMyHealthTM for patients of all North Shore-LIJ Health System hospital locations as well as select Medical Group practices. For a sortable Excel file of participating Medical Group practices, download the file by clicking here.
Please note, depending on your computer settings, the file may either open automatically or be downloaded to your downloads folder.
Additional physician practices will also have their records available through FollowMyHealthTM. Check back often for updates.
For Minor Patient
If you are the parent or legal guardian of a North Shore-LIJ Health System patient who is under the age of 13, you can request proxy access to your child’s health information in the FollowMyHealthTM patient portal. Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s health care team. When your child turns 13, you will no longer be able to view any new information about your child in the portal.
At this time, proxy access is not available for children between the ages of 13 to 18 years old.
For Adult Patient
Through the FollowMyHealthTM patient portal registered users can request online access to medical information for an adult in their care who has impaired decision-making abilities. Proxy access enables you to view the adult patient’s health information in the portal and, in some instances, communicate through the portal with the adult patient’s health care team.
Enjoy the convenience of accessing your medical records and health information while away from your computer via your iPhone/iPad or Android device, including fax and email capability. If you currently use the FollowMyHealthTM patient portal to connect to our hospitals on your computer, this app extends that accessibility to your mobile devices! Use your same login to access your Universal Health Record account with your mobile device or tablet and take control of your health and wellness.
What is FollowMyHealthTM?
FollowMyHealthTM is a patient portal, made available by North Shore-LIJ Health System, to provide a convenient way for patients to access their personal health information from any computer or mobile device with internet access.
FollowMyHealthTM is used by hospitals, doctor’s offices, and health systems across the country to provide patients with their personal health records, which enables them to take a proactive role in managing their care. FollowMyHealthTM is available online 24/7 via any computer, mobile device, tablet or smart phone.
North Shore-LIJ patients will encounter the FollowMyHealthTM name when receiving emails about the FollowMyHealthTM website. Patients registered for the FollowMyHealthTM patient portal can also download and use the FollowMyHealthTM app for iPhone/iPad and Android.

Can I access my child’s health information through FollowMyHealthTM?

How do I access my account from my mobile device?
You can install the free FollowMyHealthTM app from the iTunes App Store or Google Play store. Once you have the app, you can choose to specify certain notifications be sent via text message to your mobile phone. Text messages will not contain protected health information.

With FollowMyHealthTM you can:
